California Choice/Kaiser Permanente Choice: January 4th submission to the carrier by January 5th.

Health Net: January 5th for submission to the carrier by January 6th. January 13th for 15th of the month submission date.

*Groups enrolling under Enhanced Choice, Silver Choice or Health Net Options multi-plan option packages or on any HMO plans are not eligible for a 15th of the month policy effective date (even if they are coming off of a 15th of the month billing).

SHARP: December 30th for submission to the carrier by January 2nd.

UHC: January 6th for submission to the carrier by January 9th. January 13th for a 15th of the month submission date.
*Any group enrolling with an HMO, under a Multi-Plan option package or enrolling 5 or more employees is not eligible for a 15th of the month policy effective date.
