Aetna: May 4th for submission to the carrier by May 7th.
May 11th for 15th of the month submission date.
*A fifteenth of the month effective date will be allowed only if the group had prior carrier coverage ending on the fifteenth of the month or had no prior coverage. Proof is required.
- Blue Shield: May 4th for submission to the carrier by May 7th. May 11th for the 15th of the month submission date.

California Choice/Kaiser Permanente Choice/HSA California: May 3rd submission to the carrier by May 4th.

*Groups enrolling under Enhanced Choice, Silver Choice or Health Net Options multi-plan option packages or on any HMO plans are not eligible for a 15th of the month policy effective date (even if they are coming off of a 15th of the month billing).
