NOTE: These cutoff dates include weekends and holidays
*A fifteenth of the month effective date will be allowed only if the group had prior carrier coverage ending on the fifteenth of the month or had no prior coverage. Proof is required.

*Groups enrolling under Enhanced Choice, Silver Choice or Health Net Options multi-plan option packages or on any HMO plans are not eligible for a 15th of the month policy effective date (even if they are coming off of a 15th of the month billing).
*Any group enrolling with an HMO, under a Multi-Plan option package or enrolling 5 or more employees is not eligible for a 15th of the month policy effective date
